Bone regeneration with platelet-rich fibrin following enucleation of periapical cyst. Clinical case report

Background: The Periapical cyst is a common inflammatory lesion of the jaws; it is generally asymptomatic, it can reach a significant size and cause bone destruction; it can also sometimes cause displacement of adjacent teeth.
Objective: To present the case of a patient who, after excision of a periapical cyst, was treated with platelet-rich fibrin treatment combined with a xenograft, achieving the teeth preservation.
Case report: We present the clinical case of an asymptomatic patient who attended to a dental clinic with a failed endodontic treatment due to a defective apical seal of tooth number 31; on clinical examination, soft tissue without bone support was observed at teeth 31 and 41, on radiographic examination, a well-defined hypodense radiolucency was revealed with disruption of the buccal cortical plate and mucosal involvement. The lesion was treated surgically by excision of the periapical cyst, apicoectomy, and bone regeneration with xenograft and platelet-rich fibrin (PRF). At the one-month postoperative check-up, adequate healing process and absence of symptoms were observed. At the postoperative follow-up after one month, adequate healing and absence of symptoms were observed.
Conclusions: This case illustrates the efficacy of platelet-rich fibrin therapy combined with xenograft for the treatment of bone defects caused by a periapical cyst and the preservation of teeth adjacent the lesion.
