Post-dengue Guillain-Barré syndrome. Case report

Abstract

Background: Guillain-Barré syndrome is an acute inflammatory demyelinating polyradiculoneuropathy of unknown origin, it appears as a post-infectious disease caused by arboviruses, with emerging evidence of dengue as a trigger for this neurological complication.

Objective: To illustrate the clinical course, diagnostic and therapeutic challenges of the association between dengue and severe Guillain-Barré syndrome, likewise to contribute to the literature about the subject.

Case report: We present the case of a 27-year-old patient with a history of dengue virus infection two weeks before; he had rapidly ascending weakness, tetraplegia, facial diplegia and respiratory failure. Nerve conduction studies of peripheral nerves confirmed a demyelinating polyneuropathy; cerebrospinal fluid showed albuminocytologic dissociation. He was admitted to the Intensive Care Unit, mechanical ventilation, intravenous immunoglobulin (IVIg) 0.4 g/kg/day for 5 days, multidisciplinary support. After 7 days of mechanical ventilation, successful weaning was initiated.

Conclusions: This case reinforces the association between dengue and severe Guillain-Barré syndrome; it highlights the importance of early diagnosis, intensive care management, and the need for surveillance in endemic areas.
