Sr. Editor:

Los técnicos y profesionales publican los resultados de sus investigaciones para que sean leídas y analizadas por la comunidad interesada en dichos temas.

La sesión Cartas al Editor propicia el debate de artículos publicados y favorece el intercambio de opiniones entre escritores, editores, revisores y lectores.

Ocasionalmente en la redacción de artículos científicos se omiten o sintetizan exageradamente aspectos esenciales del diseño de la investigación que provocan inquietudes o interrogantes en los lectores ¹.

Con satisfacción hemos leído el artículo de Cabreales y cols ² publicado en la Gaceta Médica Espirituana; artículo que muestra la efectividad de la adición de sulfato de magnesio al analgésico, para el bloqueo del nervio ciático en la ciatalgia aguda; se comentan los siguientes aspectos relativos al mismo que no se encuentran declarados en el texto del artículo:

• no se especifica la causa de la ciatalgia aguda de los pacientes que integran la muestra, (diferentes causas pueden producir diferentes resultados)
• ¿se trata de un experimento o de un estudio caso control?
• se realiza un muestreo intencional por criterios, sin embargo se distribuyen los pacientes de forma aleatoria en dos grupos
• no se precisa si se informa a los pacientes las características del estudio y las  posibilidades que tienen de ser incluidos en el grupo de estudio o de control
• se declara que es un estudio a doble ciego, no se describe su diseño, sin embargo el volumen de solución administrado para la analgesia es diferente en ambos grupos (Grupo B: 15 mL de bupivacaína 0,25 % y Grupo BS: 15 mL de bupivacaína 0,25 %, más 5 mL de sulfato de magnesio como promedio)
• ¿quiénes prepararon las soluciones a infiltrar en los bloqueos?;
• el estudio se desarrolló durante dos años, ¿los bloqueos fueron realizados por la misma persona? ¿”ciega”?
• ¿quiénes evaluaron el dolor mediante la escala analógica visual (EAV)?
• hay confusión en la oración “Se consideró como tiempo de analgesia efectiva cuando el valor en la EAVN era ≤ 3 o el paciente no requirió analgesia de rescate.”
• tampoco es comprensible la siguiente oración “La información fue recogida en la historia clínica anestésica y en un formulario, el cual se confeccionó a partir de modelos utilizados y validados en investigaciones científicas durante la fundamentación teórica dando.”
• hubiera sido interesante para valorar el resultado del tratamiento inicial, la evaluación de la intensidad del dolor según la EAV, antes de la aplicación del primer bloqueo del nervio ciático.

Por el interés y la importancia de la investigación, se consideran necesarios ulteriores debates y comentarios al respecto.

Letter to the editor

Dear Editor:

Technicians and professionals publish their research results to be read and studied by the community interested in the topics.

The section ‘Letters to the Editor’ favors the debate about the published articles and the exchange of opinions among writers, editors and readers.

Occasionally, in the process of writing the scientific articles, some essential aspects of the research design are excessively summarized provoking questions and concern to the reader ¹.

We have read with satisfaction Cabrales and co-authors’ ² article published in Gaceta Médica Espirituana, which shows the effectiveness of the addition of magnesium sulfate used as analgesic to block the sciatic nerve in acute pain. The following aspects about it are not commented on in the text of the article.

• It is not specified the cause of acute sciatalgia in the patients’ sample (different causes may produce different results). 
• Is it about an experimental study or a case-control study?
• An intentional sampling criteria is followed, however, patients are distributed at random in two groups.
• It is not argued if patients are informed about the features of the study and the possibilities they have of being included in the study group or case group.
• It is stated that the study is double-blinded and the design is not described, nevertheless, the volume of the solution administered as analgesic is different in both groups (Group B: 15 mL of Bupivacaine 0.25 % and Group BS: 15 mL of Bupivacaine 0.25 %, plus an average of 5 mL of magnesium sulfate)
• Who prepared the solution for the infiltrations?
• The study was developed during two years. Were the infiltrations done by the same person? Blind?
• Who assessed pain with the visual analogic scale (EAV)?
• The sentence “It was considered analgesic effective time when the value of EAVN was ≤3 or the patient did not need a recue anesthetic” is confusing.
• Neither is understandable the following sentence: “The data was collected on the patient’s anesthetic record and on a form devised taking into consideration validated forms used in research studies referred to in the theoretical framework.”
• It would have been interesting to assess the result of the initial treatment, and pain intensity according to EAV, before giving the first blockade to the sciatic nerve.

For the sake and importance of the paper, further debate and comment on the article are necessary.
